Social Prescriber Template

Dear Colleague,
Please find below the Social Prescriber Template and associated reports. The attached files have been zipped to ensure they do not corrupt when emailed across, they will need to be saved to your desktop and extracted, then imported to SystmOne. This process will take approximately 10 minutes due to needing to add things on to the template, as explained in the guides attached.

Instructions on how to import the template and reports can also be found in the attached; as well as instructions on how to add the template to your clinical tree and/or toolbar. You may need to ask for assistance from the IT person or Practice Manager/Deputy within the practice(s) you are supporting.

You will also find attached screenshots of the template, so you can see what it will look like after you have taken the steps to import it.

Please also keep in mind that the PAM tool requires a licence to use; which is not something I can assist with.
